The privacy posture for clinical notes
Clinical conversations need a higher bar. Recordings and transcripts are encrypted in transit and at rest (hosted on Supabase), scoped to your account, and never used to train models.
Clinical templates turn the transcript into a SOAP note, with a human-review step before anything is filed. You decide whether a session is summarised at all, and can delete a recording and its transcript any time.
The Therapy Session template (SOAP + AI hybrid)
The template includes the structure clinicians actually use:
- Session metadata: patient initials/ID, date, provider, modality, duration
- AI session summary: one line takeaway plus one paragraph executive summary
- Subjective (S): patient self report, mood, symptoms, sleep, appetite, stressors
- Objective (O): observable findings, affect, speech, behavior, orientation
- Assessment (A): diagnostic impression, risk assessment (SI/HI), progress vs goals
- Plan (P): interventions, medication changes, referrals, therapy tasks, follow up dates
- Medications: current meds, doses, changes, rationale
- Diagnoses: DSM/ICD codes
- Safety and risk management: safety plan, emergency contacts, hospitalisation considerations
- Next appointment: date, time, modality
- Audit trail: human review flag, reviewer name and timestamp, AI version

Consent
Recording any clinical session requires patient consent in most jurisdictions. Notabium does not make that decision for you. It records when you click Record. We strongly recommend getting written consent before the first session you intend to record.
